• Items marked with asterisks (**) are required.
• After completing the form, use the submit button to send your order to us.  Print a copy of the completed order form for your records.
• You will receive a confirmation page verifying that your order has been received.  If you receive any page other than a confirmation, please resubmit your order.  The submission process may take up to a few minutes depending upon the speed of your connection and other factors.
• Upon receipt of your order, will will contact you via email and/or by phone to:
  1. Verbally confirm receipt of your order;
  2. Advise you of product availability, cost and applicable shipping charges;
  3. Arrange for payment and delivery.
• Merchandise that must be special ordered is non-returnable.  All special order sales are final.
